Summary
Follistatin-344 (FS344) is a myostatin-antagonizing protein investigated for increasing skeletal muscle mass; transgenic expression of human FS344 increased lean muscle mass and induced myofiber hypertrophy in pigs via reduced Smad2 phosphorylation and increased Akt signaling, a purely preclinical finding [4]. There are no controlled human interventional trials in the provided literature. Instead, the human-relevant reports concern safety harms and illicit use: a retrospective case series documented central serous chorioretinopathy (subretinal fluid and visual symptoms) in people self-injecting high-dose follistatin-344, with recurrence in those who injected repeatedly [3], and analytical work was developed to detect black-market FS344 as a doping agent [1,2]. Evidence for follistatin-344 as a legitimate administered therapeutic is preliminary, with the only human data describing adverse effects from unregulated use rather than demonstrated benefit.
