Summary

Plecanatide (Trulance) is an oral, minimally absorbed guanylate cyclase-C receptor agonist and a synthetic 16-amino-acid analog of human uroguanylin [2,3,4]. It stimulates guanylate cyclase-C receptors to increase chloride and bicarbonate secretion into the intestine and to prevent sodium absorption, drawing water into the lumen, which accelerates intestinal transit, softens stool, and eases defecation [4]. It received its first global approval in the USA in January 2017 for adults and is used to treat chronic idiopathic constipation and irritable bowel syndrome with constipation [2,3]. Because the drug and its active metabolite are not measurable in milk, it is not expected to cause adverse effects in breastfed infants, and it has not been linked to liver enzyme elevations or clinically apparent liver injury [1]. The human evidence supports plecanatide as an established, approved therapeutic with a favorable safety profile.
